Step 1: Schedule your I693 Exam
SIMPLY CLICK THE "SCHEDULE I693 EXAM ONLINE" BUTTON BELOW TO SCHEDULE. Alternatively, you may call or text 513-462-6773 to schedule an exam, but online scheduling is most efficient. You may also email us at info@midwesthealthgroup.org to schedule or ask questions. We will need your name, email address, date of birth, phone number, and street address to get you scheduled.
After scheduling, you will receive emails with all of the information you need to proceed, including your invitation to the video visit, medical intake forms, patient portal invitation, and invoice.
You will click the link to join the video visit when it is time for the video visit.
Step 2: The video visit process
During the video visit, the civil surgeon will obtain medical history, review vaccine/medical records and answer questions about the I693 process.
The civil surgeon will discuss and order any necessary tests and vaccines during the video visit.
The applicant will then be scheduled for the necessary blood and urine testing, as well as vaccines, all during the video visit.
The in-person visit is scheduled during the video visit and will take place typically 5-7 days after the video visit.
Step 3: The in-person office visit
The applicant will come into the office after having completed the video visit, all obligatory tests and vaccines.
A physical examination will be performed.
The I693 form will be completed and the sealed original I693 form will be given to the applicant at the conclusion of the in-office visit.
The applicant will be given a copy of all test results and an unsealed photocopy of the completed I693 form for their personal records.
THE PROCESS IS COMPLETE AFTER STEP 3 and the entire process takes one week or less.
Any revisions or updates of forms needed within one year of the initial form completion are free to the applicant.
The cost of the civil surgeon portion of the exam is $300. This cost does not include the cost of tests and vaccines, and will not be billed to health insurance.
If the applicant has health insurance, the health insurance will be billed for the fees charged by the lab for testing. The health insurance will also be billed for any vaccines needed. The cost of the civil surgeon service will not be billed to the health insurance company. It is an out of pocket expense that can be paid with HSA/FSA cards, credit/debit cards, or cash.
If the applicant does not have health insurance, the applicant will be responsible for the costs of the civil surgeon services, the lab test costs, as well as any additional testing needed for abnormal results (for example, a chest X-ray is needed if the initial tuberculosis blood test is abnormal), and vaccine costs. Typically, the out of pocket fees for those uninsured does not exceed $600, but will vary based on each applicant's health profile and needs.
